Are you accepting new patients?

We are currently at capacity. Please continue to check back to this page for updates. Note that we accept patients from Grey Highlands, Southgate and Chatsworth Municipalities. This is due to our funding model from the Ministry of Health and Long Term Care.

Are you a walk-in clinic?

We are not a walk-in clinic. We do offer same day appointments for urgent care for our rostered patients only. We accept calls between 8:30am and 9:30am for same day appointments. 
 

What is considered urgent?

Minor ailments such as: Sore throat, Urinary Tract Infection, Earache, Infant/Child Fever. Ongoing chronic illnesses, forms and prescription renewals should be booked with your practitioner.

 

What services do you provide?

We have a lab services onsite, physiotherapist, social workers, dietitian, chiropodist, community developer, health promoter, smoking cessation, Telemedicine, free drop in fitness area and a variety of other programs. call us today to find out more! 519-986-2222. 

 

Are you wheelchair accessible? 

Yes.

 

If I have a primary health provider somewhere else, can I come to the clinic?

We recommend that you continue to see your primary care provider at this time due to our limited capacity to take on new clients. 


If I have a doctor or NP in a neighboring community, can I utilize the programs offered at the CHC?

Yes you may. Some programs such as physiotherapy, smoking cessation, or dietitian may request that your provider fill out a referral form. Otherwise most all of the other services are available to all community members by self- referral including counseling.


Do I need a health card to access primary care at the CHC and if I don’t have one do I have to pay for services/programs?

You do not need a health care to access services at SEGCHC. We do have services that can assist you toward getting one if you want. If you have a referral for allied health services such as physiotherapy and/or chiropody and you have insurance (through your place of employment etc.) you will need to utilize a service provider who deals with insurance. We prioritize servicing clients without access to insurance. There is no charge for our programs at the clinic and these are available to anyone.

 

How do I provide feedback to the Centre (compliment or complaint)?
