Are you accepting new patients?

Presently, we are not accepting new patients but we will continue to accept completed forms for future enrollment. We have two physicians and seven nurse practitioners. If you would like to become a patient, please complete the following enrollment form and return it to the Health Centre by mail or in person. We currently accept patients from Grey Highlands, Southgate and Chatsworth Municipalities.

Are you a walk-in clinic?

We are not a walk-in clinic. We offer our walk-in patients an enrollment form to fill out. If accepted by our physicians or nurse practitioners, an appointment is then scheduled.

 

Do you have urgent care available?

We do offer same day appointment for urgent care for our rostered patients. Appointments can be made by calling the clinic between 8:30am – 9:30 am for a same day Urgent appointment.

 

What is considered urgent?

 Sore throat, Urinary Tract Infection, Earache, Infant/Child Fever. Ongoing chronic illnesses, forms and prescription renewals should be booked with your practitioner.

 

What services do you provide?

We have a lab services onsite, physiotherapist, social workers, dietician, chiropodist, community developer, health promoter, smoking cessation, Telemedicine, free drop in fitness area and a variety of programs.

 

Are you wheelchair accessible? 

Yes.

 

If I have a primary health provider somewhere else, can I come to the clinic?

We prefer that you continue to see your primary care provider if you intend to stay with that person. If you have moved to the area and prefer to get health care closer to home, please fill out an enrollment form and when you are offered a position at this clinic, you will be instructed to de-roster from your previous provider. You cannot continue to be patients of two separate providers. We do currently accept referrals for our allied health staff from other local primary providers.


If I have a doctor or NP in a neighboring community, can I utilize the programs offered at the CHC?

Yes you may. Some programs such as physiotherapy, smoking cessation, or dietitian may request that your provider fill out a referral form. Otherwise most all of the other services are by self- referral including counseling.


Do I need a health card to access primary care at the CHC and if I don’t have one do I have to pay for services/programs?

We accept people without health cards and will assist you toward getting one. We do recommend that if you have a referral for allied health services such as physiotherapy and/or chiropody and you have insurance, that you utilize a service provider who deals with insurance as we have an extensive wait list for patients with no insurance. There is no charge for our programs at the clinic and these are available to anyone.

 

How do I provide feedback to the Centre (compliment or complaint)?
