About the SEGCHC
The South East Grey Community Health Centre is a non-profit organization committed to providing primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. Our purpose is to further build on the existing support of health care through the dedicated staff in the South East Grey Community.
In addition to our main site in Markdale, SEGCHC offers programs and services at a number of locations and satellite sites to better serve our clients:
We are committed to building a healthier South East Grey Community through:
- Health promotion,
- Illness prevention, and
- Comprehensive, client-centered primary healthcare.
South East Grey Community Health Centre offers a range of community-based health services provided by a multi-disciplinary team including Physicians, Nurse Practitioners, Registered Nurses, Social Worker, Registered Dietitian, Physiotherapist, Chiropodist, Health Promoter, and Community Developer. Our priority population includes the following individuals:
- Individuals with no family doctor or whose doctor is out of region
- Seniors
- Children and youth
- The economically disadvantaged
- Individuals with mental illness/addictions
- People with mental and physical disabilities
- Pregnant women and families
We are governed by a volunteer Board of Directors consisting of community leaders, professionals and other representatives from across the geographic area served. Funding is provided by the Ontario Ministry of Health and Long-Term Care through Ontario Health West.

Accreditation
In August 2022, South East Grey Community Health Centre was awarded a three-year accreditation by the Canadian Centre for Accreditation (CCA) in recognition of achieving established CCA standards for quality in governance, management, and community-based primary health care.
South East Grey Community Health Centre is proud to be accredited by the Canadian Centre for Accreditation (CCA) which offers a third-party review process that is tailored to community-based organizations. CCA accreditation is based on widely-accepted best practice standards that promote ongoing quality improvement and responsive, effective community services.
Staff, Board of Directors, Community Members and Partner Organizations played an important part in the achievement of this success.
