The South East Grey Community Health Centre is a non-profit organization that provides primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. We are committed to building a healthier South East Grey Community through: health promotion, illness prevention and comprehensive, client-centered primary healthcare.

The SEGCHC is currently seeking a .8 permanent (30.0 hours per week) Registered Social Worker. Reporting to the Manager of Allied Health Services, you will work as a part of a dynamic, inter-professional team, providing patient advocacy, counselling/therapy and system navigation and support to priority populations of the SEGCHC. The Social Worker will participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.

Education and Qualifications:

  • Master of Social Work/Honours Bachelor of Social Work degree from recognized University.
  • 3-5 years of clinical practice experience preferred.
  • Current membership and in good standing with the OCSWSSW.
  • Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary care settings and community, with an emphasis on chronic disease management, health promotion and risk reduction.
  • Strong foundation in health promotion, health equity and the social determinants of health.
  • Excellent interpersonal, critical thinking and problem-solving skills.
  • Demonstrated ability to work independently and within an inter-professional team.
  • Efficient computer skills and experience with Electronic Medical Records/documentation.
  • Possess a valid driver’s license with access to a reliable vehicle.

2. Nurse Practitioner

We are currently seeking (1) one full time (37.5 hours per week) Primary Care Nurse Practitioner for a 13 month contract starting as soon as possible. Position salary is dependent upon experience. We offer a competitive salary along with HOOPP pension and a comprehensive benefits package.

Reporting to the Clinical Director and working as part of a Community Health Centre multi-disciplinary team, the NP’s responsibilities will include:

  • Working collaboratively with primary and allied health professionals including: physicians, other nurse practitioners, registered practical nurses, dietitians, physiotherapist, kinesiologists, social workers, chiropodist, health promoter and medical administration.
  • Working with the allied health team to develop community-based programs.
  • Working with rostered patients and working with a consulting physician.
  • Using Practice Solutions (electronic medical records system) to document all encounters and ensuring that data quality standards for charting are met.
  • Providing care at various satellite sites as required.

Education and knowledge required:

  • Must have current registration with and be a member in good standing with the College of Nurses of Ontario, with a minimum of 3-5 years’ progressive primary care experience.
  • Experience in chronic disease management strategies and program development necessary.
  • Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (Practice Solutions would be considered as asset).
  • Experience in and/or a strong understanding of the CHC model of care.

 3.  Locum Physician

The South East Grey Community Health Centre is a multi-disciplinary primary care centre providing primary care and community development services to the residents of West Grey County and the Municipalities of Grey Highlands, Southgate and Chatsworth.  We are 30 minutes to Owen Sound, 1 hour to Meaford, Thornbury and Collingwood and all that Georgian Bay has to offer; and about 1 ½ hours to all that Toronto has to offer.

We currently have an opening for a locum physician starting immediately to join our practice.  We have rural hospital site in town (Markdale) with X-Ray, Lab, 24 hour emergency services and inpatient services. Specialty Services are available in Owen Sound (Grey Bruce Health Services) with a full range of Specialists available.

We offer a very competitive salary that includes a base salary. We also offer an electronic patient record system (PS Suites) and a world class primary care facility.

The South East Grey CHC received the highest patient satisfaction rating of all of the South West LHIN CHCs. Additionally, we are the most efficient CHC in Ontario on a cost per patient served basis and cost per patient visit.  We currently have three physicians and ten nurse practitioners serving more than 8000 patients on a full and part time basis.

The primary care practice offers in-house physiotherapy, social work, dietitian (nutrition counselling) services, chiropody, and a recreational therapist.  Additionally, we offer health promotion and community development services through more than forty community-based programs.

The South East Grey CHC has three specialists working from our site in Markdale including an Internist, a Psychiatrist and an Occupation/Environmental Medicine Specialist. We work very closely with our hospital partners to ensure that patient and community needs are met.

4. Physician

The South East Grey Community Health Centre (SEGCHC) is a primary care facility located in Grey County with five sites located in Dundalk, Flesherton, Markdale and Chatsworth. The SEGCHC is dedicated to meeting the broader needs of the communities in which we serve through the development and implementation of primary care, health promotion and community development best practices.

We are currently seeking a part-time permanent Physician (22.5-26.25 hours per week), who as a valuable member of the Community Health Centre team, will provide primary care to clients and their families within a care model which emphasizes health promotion/education and empowerment.

QUALIFICATIONS

Medical degree and family residency from a recognized university.

Current CPSO license and CMPA registration.

Experience in a combination of hospital and community settings and/or public health.

Demonstrated commitment to and knowledge of community based health care.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively in a multidisciplinary setting.

Ability to work effectively with marginalized and under serviced populations.
