The South East Grey Community Health Centre is a non-profit organization committed to providing primary care services and health programs to the residents of the Municipalities of Chatsworth, Grey Highlands, Southgate and West Grey. We are committed to building a healthier South East Grey Community through: health promotion, illness prevention, and comprehensive, client-centered primary healthcare.

Working with all of our client populations, including children, youth and seniors, the Community Health Coordinator will lead and support activities, with a focus on health equity and accessibility.  The Program Assistant will advocate for patients, clients and communities to identify and ensure health and wellness needs are met throughout our extensive rural area.  Working closely with the inter-professional team, the Community Health Coordinator will partner and support health promotion activities with established summer programs and work with the Allied Health team to create new offerings.  A main focus of this job includes supporting programs related to food security, including community gardens and the YUM frozen meal program. This is an excellent experience working in a primary care health setting! The position will be 37.5 hours/per week for 16 weeks.

Qualifications:

  • Experience in a related field: health, nutrition, social work, recreation, indigenous studies
  • Between the ages of 17 and 30 years of age at the start of employment
  • Registered as a Full-Time student during the preceding academic year
  • Secondary, Post-Secondary, vocational or technical program
  • Canadian Citizen, permanent resident, or person on whom refugee protection has been conferred under the Immigration and Refugee Protection Act and;
  • Legally entitled to work according to the relevant provincial legislation and regulations.
  • Current Police Check with Vulnerable Sectors Screen

 Physician

We are currently seeking a part-full time Physician (22.5-37.5 hours per week), for a 6 month (could lead into permanent) position, who as a valuable member of the Community Health Centre team, will provide primary care to clients and their families within a care model which emphasizes health promotion/education and empowerment.

The South East Grey CHC received the highest patient satisfaction rating of all of the South West LHIN CHCs. Additionally, we are the most efficient CHC in Ontario on a cost per patient served basis and cost per patient visit. 

The primary care practice offers in-house physiotherapy, social work, dietitian (nutrition counselling) services, chiropody, and health promotion.  Additionally, we offer health promotion and community development services.

The South East Grey CHC has three specialists working from our site in Markdale including an Internist, a Psychiatrist and an Occupation/Environmental Medicine Specialist. We work very closely with our hospital partners to ensure that patient and community needs are met.

We offer a very competitive salary that includes a base salary, plus HOOPP pension and comprehensive medical benefits. We also offer an electronic patient record system (PS Suites) and a world class primary care facility. There are additional opportunities to work with our physician colleagues at Grey Bruce Health Services in Markdale in the ER and/ or in-patient if so desired. 

QUALIFICATIONS

  • Medical degree and family residency from a recognized university.
  • Current CPSO license and CMPA registration.
  • Experience in a combination of hospital and community settings and/or public health.
  • Demonstrated commitment to and knowledge of community based health care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively in a multidisciplinary setting.

 Locum Physician

The South East Grey Community Health Centre is a multi-disciplinary primary care centre providing primary care and community development services to the residents of West Grey County and the Municipalities of Grey Highlands, Southgate and Chatsworth.  We are 30 minutes to Owen Sound, 1 hour to Meaford, Thornbury and Collingwood and all that Georgian Bay has to offer; and about 1 ½ hours to all that Toronto has to offer.

We currently have an opening for a locum physician starting immediately to join our practice.  We have rural hospital site in town (Markdale) with X-Ray, Lab, 24 hour emergency services and inpatient services. Specialty Services are available in Owen Sound (Grey Bruce Health Services) with a full range of Specialists available.

We offer a very competitive salary that includes a base salary. We also offer an electronic patient record system (PS Suites) and a world class primary care facility.
