Primary Care Providers
The South East Grey Community Health Centre's Primary Care Providers include several Doctors, Nurse Practitioners, and Nurses who are dedicated to providing comprehensive primary health care to our patients. Some of our Primary Care Services include, but are not limited to:
- Regular Check-ups and Follow-ups
- Chronic Disease Management
- Immunizations
- Prenatal and Postnatal Care
- Well-Baby and Children visits
- Driver's Medicals
- Laboratory Services
- & many other services to ensure the health of you and your loved ones!
Our Patients
The South East Grey Community Health Centre provides care to patients from Grey Highlands, Southgate, Chatsworth, and West Grey Municipalities.
To access our Primary Care Providers you must be a rostered patient of SEGCHC.
At this current time our Roster is full, and we are not accepting a waitlist. We acknowledge the growing need for Primary Care Providers in the community and apologize for any inconvenience this may cause.

Schedule an Appointment
To schedule an appointment with your Primary Care Provider, contact Medical Administration during regular operation hours by calling: 519-986-2222 Ext 3
Our Medical Administration staff triages each call to determine urgency based on your health needs, and schedules an appointment within a reasonable length of time. If you require general medical advice you may request to schedule a same-day Call Back appointment to speak with a nurse.
Same Day & Urgent Care Appointments
We offer Same Day and Urgent Care appointments for those requiring a more urgent assessment. These appointments are triaged by Medical Administration staff and are offered each day on a "first-come, first-serve" basis due to limited availability.
To schedule an Urgent Care appointment please call Medical Administration
between 8:30am - 9:00am during operation hours.
Reasons for an Urgent Care or Same Day appointment may include:
- Acute cough, cold, fever, sore throat, congestion, shortness of breath etc.
- Ear infections
- Urinary tract infections (UTI)
- Pink eye
- Rash
- Mental health concerns
- Dizziness or headache
