Registered Midwives - PRAM Program
The Perinatal Rural Access Multidisciplinary (PRAM) Program is new to the South East Grey Community Health Centre and is hosted by our very own Registered Midwives (RMs), Katie Lyons & Emily Eby.
Who we Are
Through the Ministry of Health expanded midwifery care model funding, this program focuses efforts to support local families through their pregnancy and parenthood journey while collaborating with the SEGCHC team.
Unlike Registered Midwives (RMs) working in a Midwifery Practice Group setting, the SEGCHC’s RMs do not provide care for labour and birth, but instead will provide clinical care, support, and education through different stages of your pregnancy and postpartum journey to help your family get off to a great start.
The SEGCHC’s RMs will share their expertise and coordinate high quality care close to home, while prioritizing you and your family’s individual needs.

What we Provide
Prenatal Care
- Access to evidence based and person-centered prenatal care close to home (clinic locations in Markdale and Dundalk), in collaboration with local obstetric specialists, for any pregnant person in the SEGCHC catchment area, (whether or not you have a Primary Care Provider (PCP) at SEGCHC)
- The PRAM Program has partnered with Dr Yeates, Dr O’Brien and Dr Zimmerman (OB/GYNE team) at CGMH in Collingwood so that you can receive your prenatal care locally up until delivery.
- Other Obstetric providers in the surrounding areas will assume your care in the third trimester. We can request a shared care arrangement for people who have some difficulty accessing transportation.
- Individualized birth and newborn education/preparation sessions in the third trimester
- Access to SEGCHC Allied Health services and collaboration with other programs and services to provide wraparound care, depending on individual needs.
Postnatal Care
- Home Visits: We provided the option of home visits in the first few weeks after the baby is born to help set families up for success. These visits take the place of having to take your baby to a clinic to see your PCP within the first couple of weeks. The RMs will provide clinical assessments for both mom and baby, mental health screening, and general support and encouragement, as well as referrals to other community agencies as needed.
- Breastfeeding Support: The RMs will assist you in your own environment to help optimize latch & position. We will provide education and support to increase your knowledge and confidence in breastfeeding. The RMs can also assist with various infant feeding issues.
- Well woman (contraception, mental health support/screening, family planning) and well baby (routine well baby visits, immunizations, and developmental screening) care up to 18 months postpartum if you don’t have access to a local primary care provider.

Virtual Support
Those in our care can contact us directly (text or call) with non-urgent questions and concerns on the PRAM Program Line. The RMs are available Tuesday – Friday excluding holidays, and will do their best to get back to you within 2 working days.
We hope this will help to avoid unnecessary ER visits, reduce anxiety, and make you feel empowered in your parenthood journey. This service is available in the prenatal and postnatal periods for people in our care.
